Sports Physicals

Mobile Medical Services will be doing sports physicals for the 2014-2015 school year at Westlake High School on Wednesday, May 28.  The physicals will be conducted for a cost of $20.00 during the hours of 7:30 AM – Noon and will take place in the Athletic Trainer’s office.  Payment (cash, or check made payable to “MMS”) and completed forms will be required for your child to receive a physical at this time.  Forms are available on the Athletic web page, or by stopping in the Athletic Office during regular school hours.  If you have any questions, please contact Kathy Mackenzie at 440-250-1038.

OHSAA Adopts New Digital Physical Form

Medical research states 75% of sport related injuries can be reduced with PPE, and that the medical history portion is the most valuable section. Using PrivIT e-PPE will provide physicians and athletic trainers with a more detailed report prior to physicals and sport participation, which will help to identify health risk areas for each student. By improving the effectiveness of the screening process, the risk of injury will be reduced.  An additional benefit is that the medical history data you supply today can be transferred from year to year – just make a few updates to your record - no more filling out new forms each year.  The goal of the OHSAA, starting with the 2015-2016 school year, will be phasing out the paper form and PrivIT e-PPE will become the standard PPE.

PLEASE NOTE THAT PAPER COPIES ARE STILL AVAILABLE ON OUR WEBSITE OR IN THE ATHLETIC OFFICES AT WHS AND LBMS. Click here to use paper forms.
